Lucentra Care, LLC provides Applied Behavior Analysis (ABA) therapy services and related administrative, clinical, and caregiver-support tools. Our services may include intake coordination, therapy scheduling, caregiver communication, clinical data collection, progress tracking, and care team communication.

Use of our website or application does not guarantee eligibility for services, insurance coverage, authorization approval, or clinical acceptance into services.

2. Use of the Website and Application

You agree to use the Lucentra Care website and application only for lawful and appropriate purposes. You may not use our website, application, or services to:

• Submit false, misleading, or inaccurate information;
• Interfere with the security or operation of our systems;
• Attempt unauthorized access to any account, system, or data;

• Misuse communication tools or contact forms.

5. No Emergency Services

Lucentra Care’s website, application, email, text messaging, and online communication tools are not intended for emergencies.

If you are experiencing a medical, mental health, safety, or life-threatening emergency, call 911 or seek immediate emergency assistance.

6. Communication Consent

By providing your contact information, you authorize Lucentra Care to contact you using the information you provide, including by phone, email, or voicemail, for service-related, administrative, intake, scheduling, care coordination, billing, or support purposes.

Lucentra Care will send SMS/text messages only to individuals who opt in to receive SMS messages, such as by checking the SMS consent box on a Lucentra Care web form.

You agree that information you provide to Lucentra Care is accurate and that you have authority to provide such information.

7. SMS/Text Messaging Terms

Lucentra Care, LLC may send SMS/text messages to individuals who provide their mobile phone number and opt in to receive text messages from Lucentra Care.

By checking the SMS consent box on a Lucentra Care web form, you agree to receive SMS messages from Lucentra Care related to intake follow-up, appointment coordination, appointment reminders, service updates, care coordination, account support, and administrative communications.

Message frequency may vary.

Message and data rates may apply.

You may opt out of SMS messages at any time by replying STOP, STOPALL, CANCEL, END, QUIT, UNSUBSCRIBE, OPTOUT, or REVOKE. After opting out, you may receive one final confirmation message.
